Health Insurance
Health insurance has become increasingly complicated. We’re here to help. We will bill your visit directly to your insurance provider for all covered services if you supply us with updated proof of insurance prior to each visit. We will need to verify your eligibility prior to your visit; otherwise, services will be billed as out-of-network. Co-pays and fees for non-covered services are payable at the time of service.
In-Network Vision Care Providers Are the Following
* We are not in a network with Davis Vision or EyeMed.
In-Network Medical Providers Are the Following

* We are not Medicare Advantage Providers, but we may be able to help you find Vision Co-Insurance associated with your Advantage Plan.
We Would Be Honored to Be Your Eye Care Provider

Insurance Claims
We are happy to file in-network claims for you, but the remaining balance is your responsibility. Any amount not covered by your insurance policy is due after your first invoice from our billing department. Please note that some services provided are not covered by some insurance. We will do our best to explain your coverage at the time of service.
No Insurance, No Worries
I don't believe that Big Box Stores and Online Retailers equal better prices or better products. We don't "sell" our patient's glasses or contacts. We educate our patients on what products work best for their optical needs and let you decide. We constantly compare our pricing with other retailers for both glasses and contact lenses to ensure you are getting a fair price for quality products.
Clarification of Eye Insurance Coverage
It is the patient’s responsibility to know their insurance benefits and financial obligations. It can be confusing, especially with eye care. We will assist you with this as much as we can, but we must follow Federal guidelines. We must bill your insurance based on the facts of your case, not based on your coverage. For instance, we cannot bill two insurances for services on the same day or split services between insurances on the same visit.
Medical Insurance
covers services related to eye diseases and ocular symptoms (glaucoma, macular/ retinal problems, eye infections, eye discomfort, watery eyes, injuries, post-op symptoms, etc). These plans include Cigna, Blue Cross Blue Shield (BCBS), United Health Care (UHC), Aetna, Oscar, Ambetter, Medicare and others. We are not in-network on all plans.
Vision Insurance Includes
Vision Service Plan (VSP), EyeMed, Vision Care Direct (VCD), Heritage, Spectera, UHC Vision, Davis and Superior. We are not in-network on all plans.
